Care Management Program Coordinator

Summary

JOB SUMMARY
Coordinates events, marketing services, trainings, compliance, completion of narratives and reports; acts as a liaison with AHS providers, internal and external providers or entities and potential funding sources.

DUTIES & ESSENTIAL JOB FUNCTIONS

NOTE: The following are the duties performed by employees in this classification, however, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.

  • Assists with any program marketing collateral materials in various approaches (e.g. intranet, internet, social media and publications).
  • Assists with the oversight of projects and initiatives for the department; communicates project status to team and business owners throughout the duration of the assignment
  • and post implementation.
  • Designs, develops and implements work flow procedures and policies, and revises
  • procedures and manuals, ensures the availability of current information by collaborating with CM leadership Team.
  • Develops and maintains service delivery program(s)/projects under CM Leadership; assists with day-to-day operation of the program including scheduling, delivery and evaluation of program services.
  • Documents and reports Care Management programs activities in AHS systems.
  • Organize and develop training for program staff and/or volunteers including recruitment and retention efforts.
  • Provides consultation and technical assistance to internal or external partners (e.g. health care providers or other entities) to define or modify industry standards and best practices.
  • Recommends and facilitates implementation of procedural and operational changes to maximize service delivery, consumer satisfaction and potential funding opportunities.
  • Serves as a Liaison with AHS and community service providers, volunteers, community organizations to enhance service delivery and collaborative efforts on behalf of the program and the AHS patient population.
  • Support programmatic activities including but not limited to leading program meetings, developing correspondences and presentations, compiling and maintaining accurate records and files, supporting program staff schedules and developing or updating program narratives or vignettes.
  • Supports on boarding processes for all new staff or volunteers.

MINIMUM QUALIFICATIONS

Required Education: High school diploma or G.E.D.

Preferred Education: Bachelor's Degree.

Required Experience:

One year of full-time professional level or work in program administration of direct service delivery to clients and/or project management supporting clinical staff.

Preferred Experience:

Experience in a project management and planning role for clinical staff and AHS consumers.

Preferred Licenses/Certifications:

Certified Associate in Project Management (CAPM) or Project Management Professional (PMP) certification.

PAY RANGE

$28.19 to $46.95 per hour

The pay range for this position reflects the base pay scale for the role at Alameda Health System. Final compensation will be determined based on several factors, including but not limited to a candidate's experience, education, skills, licensure and certifications, departmental equity, applicable collective bargaining agreements, and the operational needs of the organization.
